Our Story
Blanchard Valley Medical Associates was founded in
1974 by Dr. William Kose. As our community has grown, our practice has
grown from one physician to a full-service multi-specialty medical
facility. Today, Blanchard Valley Medical Associates has 16 office
physicians in 7 specialties.
Additional providers include physician assistants, nurse practitioners,
and clinical pharmacists in accomplishing our goal of providing the best
possible care to our patients.
BVMA employs approximately 140 support staff serving our patients in a
team concept pulling together all of our departments to always make the
patient our priority.
Specialties & Services
Our highly trained physicians are
certified to care for patients over the age of 12. Preventative care
and health education are stressed, in addition to the care of acute
and chronic illness. We have specialists trained in cardiology, dermatology,
endocrinology, gastroenterology, pulmonary medicine and rheumatology. We
offer services including pulmonary function testing, audiograms, laboratory
testing, x-ray, stress testing, 2D cardiac echocardiography, EMG, pacemaker
analysis, thyroid scan, nuclear cardiology, sigmoidoscopy, mineral bone
density testing, diabetic education management and full body phototherapy.
Everyone in this practice operates as a
team member. All are trained professionals, and we take pride in
providing our patients with comprehensive healthcare in an
environment where our physicians in each specialty can confer to give
optimal patient care.
Billing & Insurance
Our charges for medical services are
based on the nature of the procedure and the amount of time and
professional skill involved. We request that charges be paid at the
time of service. We will prepare and submit all primary and secondary
claim forms on your behalf. In preparation to submit these claims, we
will need current copies of all your insurance cards. You will be
reimbursed for any credits to your account once insurance has paid.
If payment is not made on the day of service, then charges for
medical services are due and payable upon the receipt of your monthly
statement. We accept cash, personal check, Visa, Mastercard and
Discover.
We have participating contracts with
Medicare, Aetna, Anthem, Medical Mutual of Ohio, OhioHealth Group,
PHCS, DCA and Humana. If you are covered under any of these
companies, we ask that you pay for all co-payments and deductibles at
the time of service. We do not participate with any HMOs.
Past-due accounts, on which no payment
has been received (or contact made), will be referred to our
collection agency. If unusual circumstances make it impossible for
you to make payment according to the above schedule, we invite you to
discuss the matter with our business office. This will avoid
misunderstanding and enable you to keep your account in good

New Patients
In preparation for your first visit, we
ask that you pre-register with our office by calling 419-424-0380,
extension 3208 as soon as possible. Please have your insurance cards
available when you call. In addition, we ask that you bring the
following to your first appointment:
- All medical insurance cards;
- All medication you are currently taking;
- X-rays or requested records from
your primary care physician (unless your physician has sent them
over directly prior to your appointment).
Appointments
We will do our best to keep our
appointment schedule. However, please understand that not all
patients require the same amount of time with the doctor and that
emergencies do occur, so some delays are unavoidable. We will do our
best to keep you informed of delays. Your patience in these
situations will be greatly appreciated.
If you are unable to keep an
appointment, please notify the office at least 24 hours in advance if
possible. This courtesy on your part makes it possible to give an
appointment to another patient who desires to see the physician.
